Student Health Profile Updates for 2026-27 School Year

Dear Porter-Gaud Families,

It is time to complete all new and returning student health profiles for the 2026-2027 school year. You can access SchoolDoc by logging into the MyPG parent portal, navigating to the Resources tab, and clicking on the SchoolDoc icon. For instructions on how to log in to MyPG, please click here or email helpdesk@portergaud.edu. You will continue to receive reminder emails from SchoolDoc throughout the summer until all required information has been completed.  

Once you’ve logged into your SchoolDoc Account:

• Follow the instructions and complete the health history for your child. Alerts will appear for any missing required information.

• Please thoroughly review the information that was provided last year, make any necessary changes, and complete all information in its entirety. 

• If your child takes prescription medication at school, an authorization to administer such medication, completed by their healthcare provider, must also be provided each school year.

• You may return to SchoolDoc.com at any time to make changes/updates to your student’s health information before school begins.

• Families are expected to complete all health profiles before the first day of school on August 13, 2026.

• If your Middle or Upper School child is participating in a school sport, the Athletic Department will share information about completing athletic eligibility forms later this week. Please note that new sports physicals and authorizations are required for each school year.
