Records Request Form
Transcripts are available to current and former students upon request. Transcript requests for alumni, or former students over the age of 18 must be requested by the student who attended Porter-Gaud School. Requests will be processed within three to five business days.
***CURRENT SENIORS*** All transcript requests for Colleges and Universities must be directed to College Counseling, jharris@portergaud.edu. Do not use this form.
OFFICIAL TRANSCRIPTS are a comprehensive record of all academic courses, grades and credits completed in grades 9-12 and made official by signature and school stamp/seal. Method of delivery can be mailed in a sealed envelope directly to the institution/business via USPS, or sent electronically. USPS is the preferred method. Official transcripts are shared with colleges, universities, scholarship providers, transferring schools. Official transcripts are also provided to confirm education for professional employment opportunities, teacher or nursing licensure and other credential verification.
UNOFFICIAL TRANSCRIPTS do not contain a signature or school stamp. They may be provided in paper or electronic format directly to current and former students and to parents/guardians of current students.
